Request an e-referral
For tests and specialist consultations. Referrals are issued by your family doctor. If the test was previously recommended and is noted in your records, you usually do not need to visit the clinic.
How it works
- Call reception and tell us which test or specialist clinic your request concerns.
- The doctor reviews your records and issues an e-referral or suggests an appointment if an examination is needed.
- The referral code — four digits — goes into the e-zdrowie system; you receive it by SMS or e-mail.
- When booking by phone or in person, you give that code and your PESEL number. You can also book through the mojeIKP app.
Where to have your tests
We refer patients to the laboratories and diagnostic centres we work with: Diagnostyka, Salve Medica and Bimed.
Tests ordered by your family doctor are paid for by our clinic out of its primary care funding. That is why you have them done free of charge at the laboratory or diagnostic centre we indicate. Colonoscopy and gastroscopy are the exception — those may be done at any facility holding a National Health Fund (NFZ) contract for that examination.
Messaging apps, email and other ways to contact us are listed on the page “Book an appointment”.
